The TRICARE Family Member Dental Plan (TFMDP)

The TRICARE Family Member Dental Plan (TFMDP) is a comprehensive dental plan available to family members of active duty Uniformed Servicespersonnel.

Originally, dental coverage was provided to family members of activeduty Uniformed Services personnel in the United States, Canada, District of Columbia, Guam, Puerto Rico, and the US Virgin Islands (ContinentalUnitedStates or CONUS service area). The TFMDP was extended to all othercountries, island masses, and territorial waters (Outside ContinentalUnited States or OCONUS service area).

Implementation of the overseas extension takes effect in two phases: AsofMay 1, 1999, enrolled family members can receive care for coveredservicesfrom OCONUS providers in “remote”

OCONUS locations. Effective October1,1999, enrolled family members can receive dental care in “non-remote”OCONUS locations.

OCONUS Non-Remote Location Listing*: Azores Japan Bahrain Portugal Belgium South Korea Diego Garcia Spain Germany Turkey Iceland United Kingdom Italy/Sardinia*

All other countries not listed above should be considered "Remote"Locations.

The TFMDP, like civilian group dental plans, covers diagnostic,preventive, restorative, endodontic, orthodontic, prosthodontic and oralsurgery services from 100% to 50% depending on the type of service.Familymembers are eligible for the same benefits in both the CONUS and OCONUSservice areas (although services may not be available or accessible inallOCONUS locations). Coverage and enrollment is automatically extended forTFMDP enrolled family members, and premiums remain the same.

Enrolling in the TFMDP is voluntary. Uniformed Service members who havebeen on active duty for at least 30 consecutive days and intend toremainon active duty for at least 24 months can enroll their family members in the TFMDP. Service members who are returning with their families from anoverseas duty station on PCS orders, and who will remain on active dutyfor at least 12 months after they return, may also enroll in the TFMDP.

Enrolled family members in the CONUS service area can visit anylicensed/authorized dentist of their choice; however, visiting a UnitedConcordia participating dentist will save time, money, and paperwork.United Concordia has over 50,000 participating dentists, in allspecialties, in their network.

Different procedures apply when seeking dental care in the OCONUSservicearea.

The quality of foreign provider care is not controlled by the Governmentor United Concordia or any of its agents or representatives. The factthata foreign provider has been determined to provide acceptable dental carein the past does not guarantee acceptable future service. TheGovernment’scontrol over foreign providers is limited to their inclusion in orexclusion from the OCONUS Provider List. Sponsors/family members shouldforward any complaints or concerns about foreign providerservice/qualityof care to their respective overseas lead agent.
